Job summary
An exciting Opportunity has arisen for a Band 2 Health Care Support Worker to join the team in South Pembrokeshire Hospital - Sunderland Ward.
This is a 40 bedded Ward which is part of our frailty pathway, with a focus on slow rehabilitation of the elderly. Admission are accepted from the community & acute hospitals within Hywel Dda Health board.
The team work alongside community, primary care and the acute sites to develop an integrated approach to promoting patient independence and best outcomes for discharge.
Our focus for our patients is movement and activity on the ward which will reduce the risk of deconditioning (physiological, psychological and functional decline), which will support patients to go home on discharge, which will enhance patient recovery, as much as possible from the event that led to their acute episode and maximise their independence.
Main duties of the job
The candidate should be able to demonstrate some experience with working with the elderly and understand the principles of rehabilitation nursing.
Full time or part time hours are available.
The ability to speak Welsh, to Level 1, is essential for this post.
