      Pay Range: $25.67 - $37.70 USD Hourly
      Purpose of Job:
      Improve the overall health of the communities we serve by expanding access to quality health, wellness, and supportive services through community relations, education, and promoting awareness of CHAS Health as follows:
      Essential Duties and Responsibilities:
      • Supports the daily coordination of assigned Health Equity program activities and initiatives, including communication with community organizations and clinic administrators and conducting patient recruitment.
      • Assists in the implementation of Health Equity Service Line and program activities.
      • Supports the Health Equity Manager in implementing quality improvement activities.
      • Coordinates the development of Health Equity-related trainings in partnership with the Learning and Development team.
      • Attends health equity-related events as needed to engage with and educate the community about CHAS Health.
      • Onboards new organizations to the social services coordination platform.
      • Supports the development of workflows for health equity activities.
      • Helps with program initiatives and activities, in addition to patient wellness programs.
      • Establishes, develops, and maintains cooperative relationships between CHAS Health care teams and outside agencies.
      • Performs other duties as assigned, including supporting the CHAS Health Mission and Core Values.
      Qualifications:
      Education/Experience: Previous experience in a community health care or social services setting required. Experience in social work, public health, health services administration, or similar field preferred.
      Skills: Computer skills, time management, and organizational skills required. Self-motivated with ability to work independently and as part of a team required. Excellent oral and written communications skills required. Adaptability required. Compassion for patients required. Ability to build and maintain community relationships required. Ability to work independently required. Valid drivers’ license and insurance required.
      Physical Demands:
      Must regularly move around various work environments and be able to sit, stand, and be mobile. Using hands occurs over two-thirds of the day, while reaching with arms occurs less than one-third of the day. Regularly required to climb, bend, balance, stoop, kneel, or crouch. Regularly required to communicate by talking/hearing. Must frequently lift and/or move up to 25 pounds and occasionally lift and/or move up to 50 pounds.
