Job summary

An opportunity has arisen for enthusiastic, motivated, and innovative Health Visitors to join our experienced and established skill mixed teams within Children and Family Health Surrey based either within Guildford Borough at the Jarvis Centre Stoughton Rd, or Waverley Borough at Farnham Hospital.

We are looking for a strong team player who has a real passion for working with children and families. You will need to have excellent communication and interpersonal skills, leadership qualities and the ability to work effectively with other teams and agencies. You will also have a sound knowledge and understanding of children, young people and their families and the ability and drive to deliver the Healthy Child Programme (HCP) to a diverse population across defined geographical areas around the Borough.

We are committed to the care and development of our staff and whether you are an experienced health visitor or newly qualified we offer support and preceptorship as needed. This post is subject to a probationary period.

We take great pride in nurturing our staff, children and families and in building exceptional teams. The role is Monday to Friday in which we can offer some flexibility around hours. It is a client facing role but with room for some home working based on service need.

Job responsibilities

P ri ncipal duties

Work collaboratively with the skill mixed health visiting team to ensure equity of service provision, promoting good practice and the sharing of resources.

Prioritise own work in line with areas of highest risk or client groups with greatest health need, delegating other work to the wider skill mixed team whilst providing the team with supervision and support

Facilitate good team working, through effective leadership skills delegating and supervising the work of a skill mixed team to ensure efficient delivery of the 0-5 HCP

Assist in meeting public health targets e.g. teenage sexual health, accident prevention, breast feeding, immunisation, smoking cessation and obesity

Undertake continued responsibility for the management of a caseload

Build and develop good working relationships with childrens centres, midwifery, GP practices and the wider multi-disciplinary team. Work collaboratively with all relevant agencies to influence/develop policies which will enhance health

Be responsible for organisation, prioritisation and planning of health promotion activities, by assessing local health needs and promote key health topics to meet All our health 2016 DH targets

Where appropriate work with Children and Family Health Surrey (CFHS) Nurse prescribing lead to prescribe in line with local and national policy guidelines.

Make Every Contact Count with children, and their families in a variety of community settings e.g home, children centres, nurseries

Undertake well child clinic sessions within agreed venues and children centres

Assist in the planning and where appropriate in the delivery of continence assessments

Identify and support children and families who have unmet health needs, providing support and targeted interventions and where necessary referring to specialist services

Participate in prevention and early detection of all categories of abuse in all age groups

Take appropriate action in cases of suspected or actual child abuse, working within Surrey safeguarding childrens code of practice and procedures

Attend child protection conferences/core groups as required and contribute to the development of the protection plans and decision making processes, within the multi-agency setting

Participate in safeguarding supervision

Ensure that clear concise reports are written and that organisational record keeping guidelines are adhered to

Assist in research, participate in clinical audit and evaluation of service including the monitoring of records of others within the 0-19 team

Undertake health needs assessments for children who are looked after

Support and attend group clinical supervision as per organisation policy

Actively participating in steering groups to develop HV practice guidelines

Supervise junior members of the skill mixed 0-19 team as directed by the Clinical Team Lead and undertake junior staff Personal development Review (PDR) appraisals and direct line management.

Contribute to the pre and post registration opportunities made available to students across CFHS, which may include supervision of student practice placements

Assess practice competencies of junior staff members, contributing to their training and development needs

Participate in meetings as appropriate

Be prepared to be flexible within contracted hours
