      HIV Community Liaison

      CLASSIFICATION/STATUS: Full-time, Exempt, Grant Funded

      Immediate Supervisor: Practice Transformation Director

      Supervisory Responsibilities:

      Salary: $47,840/year (pay rate set by grant specifications)

      The mission of Charles River Community Health (CRCH) is to partner with individuals and families so they can thrive and lead healthier lives by delivering the comprehensive, integrated, and equitable primary healthcare that matters most to them. This position will be responsible for participating in and/or leading key programmatic activities such as implementing program strategies, evaluating program outcomes, and ensuring full integration of the HIV Prevention Program activities at CRCH. This position will assist with developing strategies with community partners to identify outreach and education strategies in the community. Additionally, the HIV Community Liaison will work with the provider champion and Chief Clinical Officer to ensure team members are trained and facilitate ongoing education.

      YOUR RESPONSIBILITIES

      • Assist HIV Prevention Coordinator in efforts to increase HIV screenings rates, and linking patients with HIV to care within a timely manner by solidifying relationship with providers and the care teams

      • Attend initial and continuing education training programs

      • Provide informal counseling to newly diagnosed patients, or patients beginning a new therapeutic regimen

      • Support linkage to and retention in care by re-engaging patients lost to follow-up

      • Perform home visits for patients, providing emotional, informational, and tangible support

      • Make follow-up calls to patients that have an existing diagnosis of HIV, and ensure their care plans are being met

      • Like patients to community resources that need additional support for identified social drivers of health

      • Assist HIV Prevention Coordinator with facilitating trainings for providers and all other staff on HIV program

      • Integrate into care team meetings; communicate any findings between departments and support improvement initiatives

      • Support retention n care and reduce barriers by collaborating with medical, behavioral health, and social services providers

      • Participate in HIV multidisciplinary taskforce with regular monthly meetings

      • Actively support providers and clinical staff in navigating newly diagnosed patients with HIV, or those that are new to case with existing diagnosis

      • Coordinates related HIV program PDSA cycles across all service lines within the organization

      • Organize and maintain appropriate community resources and partnerships for the HIV program

      • Facilitate community-based and program referrals for vulnerable populations needing additional connection to services

      • Document care activities in the patient medical record

      • Track and follow-up on referrals in regards to program deliverables

      YOUR QUALIFICATIONS, COMPETENCIES, TRAITS

      • Bachelor’s degree in public health, health administration, or related field preferred

      • 3+ years of experience in public health or community health

      • Experience in infectious disease preferred

      • Microsoft Office experience required
