      What to Expect (Job Responsibilities):
      - Collaborate with At-Home providers and Psychiatric Consult to address patients' behavioral health needs
      - Facilitate goals of care and advance care planning discussions with patients and families
      - Manage referrals for specialized services and maintain a shared behavioral health registry
      - Screen and assess patients for mental health and substance use disorders
      - Provide short-term counseling and evidence-based treatments within the Collaborative Care Model

      What is Required (Qualifications):
      - Licensed Clinical Social Worker (LCSW) with US work authorization
      - Ability to work in varied environments, including patients' homes
      - Experience in performing comprehensive biopsychosocial health assessments
      - Working knowledge of differential diagnosis and psychosocial treatments for common mental health disorders
      - Proficient PC skills

      How to Stand Out (Preferred Qualifications):
      - Familiarity with brief, structured intervention techniques such as Motivational Interviewing and Behavioral Activation
      - Demonstrated ability to collaborate effectively in a team setting
      - Experience supporting patients with significant health and psychosocial issues
