What to Expect (Job Responsibilities):
- Assist members in housing search and placement to promote self-sufficiency and community integration
- Conduct intake interviews and assess housing barriers to determine members' needs and goals
- Develop individualized action plans that focus on permanent housing and self-sufficiency
- Advocate for members to reduce barriers in accessing affordable housing and supportive services
- Maintain accurate records and reports for each client, ensuring proper documentation of progress

What is Required (Qualifications):
- Bachelor's Degree or equivalent combination of education and relevant experience
- Minimum 2 years' experience working with underserved or special needs populations
- Familiarity with affordable housing resources and social services
- Knowledge of state and federal guidelines, including the Fair Housing Act
- Valid driver's license with a good driving record and reliable transportation

How to Stand Out (Preferred Qualifications):
- Bilingual (Spanish) based on community need
- Experience with healthcare systems and processes, including NCQA and Interqual
- Knowledge of community-specific culture
- Experience working with individuals with severe and persistent mental illness
- Current Community Health Worker (CHW) Certification or Medical Assistant Certification
