What to Expect (Job Responsibilities):
- Conduct in-home assessments on health plan members, including obtaining comprehensive history and physical exams
- Engage in end-of-life care discussions with patients and families
- Collect laboratory specimens in the home setting when appropriate
- Identify diagnoses for active medical management focused on chronic disease management
- Communicate assessment findings to inform primary care providers of potential gaps in care
What is Required (Qualifications):
- Master's degree from an accredited Nurse Practitioner or Physician Assistant program
- Active California state Nurse Practitioner or Physician Assistant license
- Active RN License and Furnishing number (for Nurse Practitioners only)
- Active Nurse Practitioner Board Certification or Physician Assistant Certification
- Valid California driver license and current automobile insurance
How to Stand Out (Preferred Qualifications):
- One (1) year of prior clinical or home care experience
- Previous experience with electronic medical records (EMR)
- Experience in the care of older adult (geriatric) patients
- Excellent administrative, organizational, and communication skills
