What to Expect (Job Responsibilities):
- Conduct in-home assessments and outpatient clinical work for health plan members
- Obtain comprehensive patient history, physical exams, medication reviews, and screenings
- Facilitate end-of-life care discussions when necessary
- Collect laboratory specimens in the home setting as appropriate
- Communicate assessment findings to primary care providers to address potential gaps in care

What is Required (Qualifications):
- Active California state Nurse Practitioner or Physician Assistant license
- Master's degree from an accredited Nurse Practitioner or Physician Assistant program
- Ability to communicate positively and effectively with patients and team members
- Strong problem-solving, organizational, and time management skills
- Valid California driver’s license and current automobile insurance

How to Stand Out (Preferred Qualifications):
- One (1) year of prior clinical or home care experience
- Experience in the care of geriatric patients
- Previous experience with electronic medical records (EMR)
- Active RN License and Furnishing number for Nurse Practitioners
