What to Expect (Job Responsibilities):
- Conduct in-home assessments on members, including medication review and health screenings
- Provide patient education by aligning care desired with the best available options
- Identify diagnoses for care management and active medical management
- Collaborate with the Regional Medical Officer and team members to create appropriate care plans
- Coordinate care with multiple stakeholders, including primary care providers and specialists

What is Required (Qualifications):
- Must be able to conduct home visits in Wake, Chatham, Johnston, and Orange Counties
- Master's degree from an accredited Nurse Practitioner or Physician Assistant program
- Active North Carolina state Nurse Practitioner or Physician Assistant license
- Active RN License and Furnishing number (for Nurse Practitioners)
- Valid Nurse Practitioner Board Certification/Physician Assistant Certification, NPI Number, DEA, and Valid BLS

How to Stand Out (Preferred Qualifications):
- One (1) year of prior clinical or home care experience
- Previous experience with Electronic Medical Records (EMR)
- Experience in the care of older adult (geriatric) patients
