      Job Description:

      Under the general direction of, and in collaboration with the physician, the Advanced Practice Provider (CRNP, PA-C) fulfilling the role of Inboxologist will provide diagnosis and treatment plans for patients within their scope of practice and clinical license as defined by applicable Federal, State, and local laws. Coordinates patient care, services, and clinical documentation and indirect provider for treating patients. Responsibilities will vary depending on whether the work is being performed in the practice, home-based, or combination of both.

      Duties and Responsibilities

      Essential Accountabilities:

      1. Reviews patients’ records, messages, results, and history to assess their current health status and develop proper treatment plans.
      2. Identifies real and/or potential health problems and identifies expected outcomes in response to management plan and reviewing patient data with physician as necessary for proper treatment.
      3. Provides guidance and coordination of care to patient and their caregivers.
      4. Provides clinical guidance and coordination of care in response to test results, medication requests, patient related health questions regarding treatment/illness.
      5. Requests appropriate diagnostic and screening procedures, interpreting the results as allowed by State Regulations and protocols.
      6. Refills and starts new prescriptions for required medication and/or treatment to meet management plan as allowed by State Regulations and protocols.
      7. Coordinates continued care for individual patients.
      8. Reinforces physician instructions as appropriate.
      9. Collaborates with patient’s primary care team in providing patient evaluations, improvement plans and patient outcomes.
      10. Protects patient confidentiality by promoting professional staff communications.
      11. Maintains a thorough knowledge and understanding of evidence-based medicine/best practice.
      12. Acts as a resource to practice providers and staff.

      NonEssential Accountabilities:

      1. Attends office meetings as required.
      2. Performs other related duties as identified.

      Qualifications

      • Notes:
        • Other combinations of formal education, training and experience may be considered.
      • Minimum Experience:
        • 3-5 years clinical experience providing patient care
      • Minimum Education:
        • Physician Assistant (PA-C) or Nurse Practitioner (CRNP)
      • Required Certification:
        • Nurse Practitioner- Registration and current license as CRNP in PA; board
        • Certified by American Nurses Credentialing Center Commission on Certification, BLS certification
        • Physician Assistant - NCCPA (National Commission on Certification of PA’s) certification
      • Physical Demands:
        • Mostly sedentary work
        • Occasionally prolonged standing or walking
        • Occasional reaching, stooping, bending, kneeling, crouching
