What to Expect (Job Responsibilities):
- Conduct patient assessments to ensure safety and appropriate provider placement
- Coordinate activities to improve outcomes for high-risk patients, including thorough chart reviews and communication with providers
- Educate patients on resources, timelines, and goals to promote accountability in their health programs
- Respond to service referrals and voice messages within designated timeframes
- Complete documentation on patient encounters and ensure compliance with HIPAA guidelines
What is Required (Qualifications):
- Minimum of 2 years of experience in a community or non-community setting
- High School Diploma or GED
- Expert knowledge of community resources and basic nutrition
- Ability to utilize computers for documentation and information access
- Strong communication skills, both oral and written
How to Stand Out (Preferred Qualifications):
- Experience with electronic medical records (EMR)
- Skills in case management, crisis intervention, and problem-solving
- Ability to analyze complex data and draw conclusions
