      Overview:

      Sentara Health is seeking a dedicated Integrated Nurse Case Manager for the Fredericksburg, VA area !

      This is a full-time, work-from-home position requires travel to conduct face-to-face home visits in the member’s homes around the Fredericksburg, VA area

      As an Integrated Care Manager with Sentara, you get to work together in an intensely collaborative way with the patients, family, and members of the medical team to provide patients with the resources they need and help prevent them from readmitting into the hospital. This position's primary duty is customizing what is best for the patients. That would include helping with things like home health, long-term care placement, home medical equipment, homeless and community resources.       Education

      • Bachelor's in Nursing preferred

      Certification/Licensure

      • Registered Nurse License (RN) - Nursing License - Compact/Multi-State License

      Experience

      • Experience working with the adult population .
      • Knowledge of community health and social service agencies and additional community resources
      • Ability to use a variety of electronic information applications/software programs including electronic medical records
      • Intermediate to Advanced computer skills and experience with Microsoft Word, Outlook, and Excel with excellent keyboard and web navigation skills
      • Exceptional communication and interpersonal skills with the ability to quickly build rapport
      • Ability to work with minimal supervision within the role and scope.
      • LTC or Nursing Home experience preferred .
      • Case Management/Care Coordination experience .
      • Previous managed care experience.
      • Experience with health promotion, coaching and wellness.

      We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. The base pay rate for Full Time employment is:$66,830.40-$111,384.00Additional compensation may be available for this role such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
