      What to Expect (Job Responsibilities):
      - Collaborate with the market Medical Director and care team to identify patients who qualify for palliative care services
      - Develop and present metrics to executive leadership as requested
      - Drive healthcare outcomes for a patient panel through both in-home and virtual care
      - Evaluate and treat symptoms affecting quality of life, such as pain, depression, and anxiety
      - Educate and support the team around palliative care, hospice care, and advance directives

      What is Required (Qualifications):
      - Doctorate Degree in Medicine
      - Board Certified in Hospice and Palliative Medicine
      - Current state medical license without restrictions and free of sanctions from Medicaid or Medicare
      - Experience with high-need Medicare Advantage and managed Medicaid populations
      - Willingness to become licensed in multiple states as required by company and patient needs

      How to Stand Out (Preferred Qualifications):
      - Experience integrating acute and behavioral healthcare and understanding social determinants of health
      - Comfort and skill in goals of care discussions and advance care planning
      - Demonstrated experience applying evidence-based clinical criteria
      - Expertise in managing medication panels for complex patients
      - Desire to practice in a forward-looking renal care environment focused on population health management
