What to Expect (Job Responsibilities):
- Collaborate with the market Medical Director and care team to identify patients qualifying for palliative care services and build a patient panel
- Evaluate and treat symptoms that may interfere with quality of life, such as pain, depression, and anxiety
- Conduct goals of care and family meetings, documenting patient/family wishes
- Develop key relationships with local providers to improve market alignment
- Educate and support the team around palliative care, hospice care, and advance directives
What is Required (Qualifications):
- Doctorate Degree in Medicine
- Board Certified in Hospice and Palliative Medicine
- Current state medical license without restrictions and free of sanctions from Medicaid or Medicare
- Experience with high-need Medicare Advantage and managed Medicaid populations
- Must have reliable transportation

- Demonstrated experience applying evidence-based clinical criteria
- Comfort and skill in goals of care discussions and advance care planning
- Experience integrating acute and behavioral health care and understanding social determinants of health
- Expertise in managing medication panels for complex patients
- Willingness to become licensed in multiple states as company and patient needs require
