- Knowledge of Word, Excel, PowerPoint, Knowledge of, and regular use of, strategic /corporate information systems and patient/service user record systems
- Knowledge and understanding of health and social care terminology.
- Experience of using EMIS, ICE, Liquid Logic, STRATA.
- Accurate keyboard skills.
- Improvement skills (or be willing to attend training and development as appropriate).
- Excellent telephone skills for dealing with calls to refer individuals to ICC Service from a variety of agencies.
- To be friendly, tactful, articulate, sympathetic and self-motivated.
- Ability to communicate with staff and individuals using services at all levels.

Job summary
An exciting opportunity has arisen for a proactive individual to work within the Community Rehabilitation Therapy Team at Copeland ICC. The post is full time, 37.5 hours per week, part time and flexible working patterns considered.
We are a friendly and supportive team that consists of OTs, Physios, HCP co-ordinators, Assistant Practitioners, hub administrators, Rehabilitation assistants and home care practitioners. We work closely with the MDT including district nurses, GP practices,Adult Social Care, third sector organisations as well as many other community services and professionals.
The post holder will work closely with the professional of the day and the hub team identifying patients that can be supported to be discharged home from hospital or to prevent admissions that are not medically required.
Main duties of the job
The Community Care Group provides a range of services to people over 18 living within North Cumbria. Its key community health focus is to provide care to prevent hospital admissions, facilitate early discharge and where possible to enable patients to manage their own conditions and remains in their own homes, enhancing both their independence and quality of life.
The professional of the day support will track patients in Copeland and know where in the system they are, whether they are in hospital, a community bed or under the care of a community health or care team. The support worker will work proactively to ensure these people are being actively managed to stay at home.
You will monitor individuals who are admitted to acute and community hospitals and actively 'pull' them through the system to facilitate timely discharge through the coordination of local services and support.
Under the direction of the practitioner of the day you will ensure that urgent referrals are managed and the best outcome for the individual is achieved in terms of caring for people in their own homes where possible.
You will facilitate access to step up / interim beds under direction of practitioner.
Once the patients needs are identified, you will take out equipment to support patients to remain safely at home.
