      Empath Health is currently seeking a Lead Registered Nurse with Home Care Home Base experience to join our team.

      This is a remote position that requires a safe, secure, and professional work environment.

      The Lead Registered Nurse processes all electronic medical record workflow from admission to discharge. Serves as a professional leader in assessing, planning, guiding and evaluating patient care. Interacts with management, physicians, inpatient and outpatient clinical staff, and others utilizing a teamwork approach.

      Position Requirements:

      • Currently licensed and registered in the State of Florida. BSN and/or Hospice certification preferred.
      • One year of recent hospice, oncology, medical/surgical, or home health, public/health experience preferred.
      • One year of Electronic Medical Records experience, Homecare Homebase software experienced.
      • Expertise in support skills, palliative care, and educational techniques.
      • Demonstrates organizational, administrative and personnel management skills.
      • Communicates clearly and effectively in writing and verbally.
      • Flexible, creative, assertive, articulate, nurturing and self-nurturing.
      • Computer skills are a necessary component of position.
      • Current Basic Life Support Certification.
      • Able to work well under stress and deadlines.

      JOB DUTIES/RESPONSIBILITIES:

      • Processes admissions, discharges, transfers, branch transfers, service location changes, team orders, level of care changes per HCHB/EMR guidelines. Reviews and edits as necessary MD orders. Completes periodic medical record review to ensure accuracy, completeness and compliance with plan of care. Ensures compliance with regulatory standards, State, Federal and others. Ensure all visit codes and branch codes comply with EMR billing.
      • Ensures workflow scheduled and approved per HCHB, agency, and Medicare guidelines. Monitors patient medical records as changes are made to ensure all necessary MD orders have been obtained, LOC is correct, service locations updated, branch transfers completed, and calendars updated as indicated.
      • Serves as a resource for all staff to assist with changes to patient medical records and troubleshooting as necessary. Provides providing timely feedback to staff regarding incorrect orders, LOC changes, transfers of all types. Assists with monitoring patient visits to ensure compliance with scope and frequency.
      • Interfaces with other departments, care groups, and clinical leaders to discuss and resolve problems and ensure the best interest of the organization is met. Communicates and collaborates with all disciplines on the team to ensure full participation of all staff in the care of the patient.

      • Maintains proficiency with EMR, Digital Care, and other computerized systems.
      • Maintains flexibility and willingness to accept new assignments based on needs of the organization. Represents Empath at events in the community.
      • Shows professionalism, treating others with dignity and respect. Avoids unnecessary conflict and provides a positive and optimistic attitude.
