Job Description

- Licensed LPN in the State of Louisiana- required
- A minimum of three years’ experience required in Quality Assurance management - preferred
- Knowledge of Hospice and/or Home Health regulations - preferred
- The ability to maintain company policies and procedures
- Experience with maintaining QAPI program according to company guide lines -required
- Experience in reviewing nurses’ notes for completeness, compliance with physicians’ orders, authorized services, and regulations.
- Experience in reviewing admission packets - required
- Experience required in reviewing all POC and recertification for: accuracy, order updates, frequency updates, goal updates, medication profile reviews, format and wording, diagnoses updates, C.N.A. Care Plan.
- Will assist with one-on-one in-services for documentation problems as needed
- Effective written and verbal communication
- Ability to travel - must have reliable transportation
- Clean background and drug screen

We are currently seeking qualified candidates for a full time Licensed Practical Nurse (LPN) Quality Assurance position in the Lake Charles area.
Job Description:
The Quality Assurance LPN is an integral part of the hospice team, and as such is responsible for assuring quality control of assigned sites. The LPN/QA will review documentation for appropriateness of care. Keeping in mind the mission of this agency, it is the responsibility of the Quality Assurance LPN to assist in assuring all patient services are delivered according to Harbor’s philosophy, concepts of care and accepted practice. And will efficiently utilize the resources of the company to provide quality care.
