      Key/Essential Functions & Responsibilities

      • Wound Care, Medication Management, G-Tube Administration, etc.

      • Report any inconsistencies in our member’s condition and family situation

      • Document reports and observations

      • Interacts with patients and families

      • Teach appropriate self-care techniques

      • Consult with Case Manager/MD in determining plan of care

      • Notify Case Manager, MD, and/or families of changes in member’s condition

      • Maintains reporting systems

      • Report abuse or unsafe/illegal practices

      • Make recommendations to improve quality of life for member

      Minimum Qualification Requirements including education, experience, licensure/certification, etc. and essential physical capabilities (e.g. lifting, assisting lifting, standing, etc.) Minimum requirements of specific positions may vary by assigned function and/or contract. Please refer to applicable regulations/contracts for all position requirements.

      All LPN must meet the following minimum requirements:

      • NYS Licensed Practical Nurse (LPN) License

      Preferred Qualification Requirements ( desired requirements beyond MQRs above)

      • Med/Surg experience

      • Experience with intellectually and developmentally disabled population

      Compensation:

      • Salary is up to 65000 USD annually

      Schedule:

      • M on, Wed, Fri, every 3 rd Sat 7pm-7am
