      Description

      POSITION OBJECTIVES: T o assist in creating and implementing life plans and managing services and supports for persons with intellectual and developmental disabilities.

      RESPONSIBLE TO: Director Life Path Planning and Management

      SPECIFIC FUNCTIONS:

      • Provide advocacy support and education to individuals with disabilities, guardians, caregivers and others as needed.
      • Assessing physical and mental wellness, needs, preferences and abilities of person with intellectual and developmental disabilities (client) and the family’s resources and existing future plans--using these to develop a tailored life/management plan.
      • Working with clients, family and friend support networks and health care professionals to put care plans in place.
      • Documenting plan implementation including tracking referrals, conducting home visits, and intervening with agencies, and interacting with family, client and community supports.
      • Documentation will also include timekeeping, note-taking, and preservation of protected health information.
      • Assessing, planning, implementing, monitoring, and evaluating programs, services and support required to meet the client’s needs through a written life plan or management contract.
      • Evaluating life plan/management plan periodically and adjusting as needed to improve outcomes for those served.
      • Following up with families and persons with disabilities regularly to measure satisfaction with plan and management services and to ensure they are still in good physical and mental health and on track with plan.
      • Schedule and coordinate appointments, meetings,
      • Assist in preparation of grant applications, accounting and reports.
      • Public speaking and presentations as required.
      • Participate in community engagement and outreach activities.
      • May be required to work some evenings and weekends.

      Qualifications


      EDUCATION: Bachelor’s degree in an appropriate area of specialization.

      QUALIFICATIONS: Must have minimum two years’ experience working in the field of developmental disabilities with an awareness of supports and services available to individuals with disabilities. Excellent written and oral communication skills. Excellent time management skills. Must have computer experience in Word, Excel, PowerPoint, Publisher, and Adobe. Must have a valid Florida Driver’s License.

      SALARY: $45,000/annual full-time employment
