What to Expect (Job Responsibilities):
- Coordinate and complete the RAI process and care management process from admission to discharge
- Collaborate with the Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews
- Ensure compliance with federal and state regulations regarding the RAI process
- Maintain accurate documentation and data entry of resident information into computerized MDS programs
- Assist with the coordination and management of daily meetings to review resident care

What is Required (Qualifications):
- Graduation from an approved RN / LVN program and state licensure
- Minimum of 2 years of nursing experience in a Skilled Nursing Facility
- Excellent knowledge of Case-Mix, Federal Medicare PPS process, and Medicaid reimbursement
- Thorough understanding of the Quality Indicator process, OBRA regulations, and Minimum Data Set
- Experience with MDS 3.0

How to Stand Out (Preferred Qualifications):
- Experience with MDS assessments and care planning process
- Familiarity with medical record review and coding of MDS
- Strong communication and interpersonal skills
