We offer three major areas of care: Long-Term Care , Short-Term Care/Rehabilitation and Memory Care .

      We are actively seeking a Long Term Care Social Worker and offering a $5,000 Sign on Bonus!

      You will join our amazing team in one of our Long Term Care Units. We are excited to launch a comprehensive program that will also include therapeutic activities and social services coordination on the unit. Come join us and lend your enthusiasm and ideas to this new program, aimed to meet the psycho-social needs of our residents through dynamic activities and compassionate social services.

      Schedule: Sunday through Thursday

      Primary responsibilities:

      • Comprehensive psychosocial assessments
      • Developing and maintaining a plan of care in collaboration with the team
      • Coordinating dynamic activity programs and documentation on the unit
      • Resource referral and contact with families of residents to provide support and/or collaboration towards goals of the family and resident
      • Crisis and therapeutic interventions
      • perform assigned resident interviews that assess each individual’s mood, behavior, and cognitive ability and write the accompanying summary of findings

      Knowledge, skills and abilities:

      • Provide education and techniques to unit staff of person-centered interventions that can be utilized to address any mood, behavioral, or cognitive concerns residents might present with.
      • Meets deadlines for time sensitive responsibilities
        Excellent follow through and communication skills
      • Ability to work with charge nurse to lead the Unit team
      • Passion for the elderly
      • Ability to engage with families, residents and team

      Qualifications:

      • Degree in Social work or related field of study
      • 2 years' experience in Human Services or applicable field
      • Prior experience in activities a plus
      • Ability manage moving priorities each day
      • Strong verbal and written communication skills
      • Computer skills for charting and documentation
      • Strong team player with the ability to have FUN!
