What to Expect (Job Responsibilities):
- Coordinate transitions of care for TRICARE beneficiaries through inbound and outbound calls
- Utilize clinical and analytical skills to identify care coordination needs and facilitate care during transitions
- Communicate with community providers, Military Treatment Facilities, and Veterans to coordinate care
- Ensure authorizations are in place to meet clinical care needs and document coordination activities accurately
- Participate in Quality Improvement Projects and report potential quality or fraudulent cases as needed

What is Required (Qualifications):
- Licensed Practical or Vocational Nurse (LPN/LVN) with a current unrestricted license in the appropriate state or territory
- Minimum of 2 years of clinical nursing experience
- Proficiency in medical data entry, written skills, and Microsoft Office
- Exceptional oral communication skills and strong organizational abilities
- 2+ years of Managed Care experience preferred

How to Stand Out (Preferred Qualifications):
- 3 years of clinical experience
- Experience working with TRICARE or military health systems
- Familiarity with Quality Improvement Projects
