      What to Expect (Job Responsibilities):
      - Coordinate intake processes for patients in a home health setting
      - Ensure compliance with state and federal accreditation requirements
      - Perform data entry accurately and efficiently
      - Communicate effectively with patients and healthcare team members via telephone
      - Provide support and information to patients and their families throughout their health journey

      What is Required (Qualifications):
      - Valid license as a Licensed Vocational Nurse (LVN) or Registered Nurse (RN)
      - Minimum of one year of experience as an LVN or RN in a medical setting, preferably home health
      - Knowledge of state and federal accreditation requirements
      - Excellent verbal and written communication skills, particularly over the phone
      - Proficient data entry skills

      - Experience in a home health environment
      - Familiarity with healthcare regulations and compliance standards
      - Strong interpersonal skills and ability to empathize with patients
      - Previous experience in intake coordination or similar roles
