      Overview

      The Manager, Case Management oversees a team of case managers, ensuring the effective delivery of services to clients/patients. The role involves coordinating patient-specific plans, monitoring case progress, and providing guidance and support to all staff. This role will supervise the daily operations and team performance to ensure compliance with Central Health and regulatory policies and guidelines.Additionally, the person in this role is responsible for applying empowerment-based prevention and intervention strategies to improve community health, making referrals to community-based resources, addressing barriers to improving well-being, offering expertise regarding psychosocial issues affecting patient care, and other case management activities.

      This is an onsite position, located in Austin, Tx.

      Responsibilities

      Essential Functions:

      • Supervises daily operations, productivity, and performance of Case Management team and provide guidance and support to enhance the team’s performance and professional development.
      • Review and monitor patient cases to ensure appropriate service delivery and adherence to care plans.
      • Ensures compliance with regulatory standards, organizational policies, and best practices in case management.
      • Conducts training sessions for new and existing staff on case management procedures, best practices, and tools.
      • Partner with healthcare providers, social services, and community resources to coordinate comprehensive care for clients.
      • Address client concerns and needs, ensuring they receive appropriate services and support.
      • Participate in the development and implementation of policies and procedures related to case management.
      • Assess staff performance regularly and provide constructive feedback to promote growth and accountability.
      • Apply empowerment-based case management strategies to reduce negative social determinants of health in highrisk populations including people experiencing homelessness, those with substance use disorders, HIV, etc.
      • Address personal and environmental issues with the patient to maximize emotional, social, and physical wellbeing and the appropriate use of healthcare resources.
      • Provide the expertise, consultation, and education in areas relating to psychosocial issues affecting patient care for individuals experiencing homelessness, substance use disorder, or other high-risk conditions.
      • Attend patient care (unit) huddles and periodic informational (clinic) huddles, meetings, patient conferences, or planning sessions. These may be related to quality assurance, patient care and other related topics within the clinic.
      • Participate in quality strategies to evaluate compliance with evidence-based guidelines, standards and to identify opportunities to improve patient outcomes.
      • Assess individual needs, strengths, barriers, and readiness to change under the clinical supervision of a board certified LCSW supervisor.

      Knowledge, Skills and Abilities:

      • Works collaboratively with all members of the organization to resolve challenges, economically, professionally, and with minimum disruption to the organization.
      • Demonstrated proficiency in the use of Windows operating system, Windows Office products, and electronic health records.
      • Ability to work independently and within a team to problem-solve and accomplish goals as per established timelines.
      • Demonstrated knowledge and competency in leading training and/or co-facilitating with a team.
      • Ability to communicate in various settings, including but not limited to group presentations and small‐scale meetings.
      • Ability to work with diverse groups of people.
      • Demonstrated knowledge and competency prioritization and time management.
      • Experience in oversight of performance metrics..
      • Knowledge of local, state, and federal regulations and laws governing health and human services activities.
      • Experience/knowledge of Joint Commission accreditation process and requirements, as well as all federal, state and local regulations and standards associated with the delivery of care in a FQHC.
      • Demonstrated understanding and/or experience working in a PCMH environment.
      • Strong attention to detail and accuracy.
      • Ability to travel frequently to other clinic sites where patients are receiving care.
      • Demonstrated success in the delivery of quality nursing practices and duties assigned to this role.
      • Excellent conflict management and mediation skills.

      Qualifications

      MINIMUM QUALIFICATIONS:

      Required Education:

      • Master's Degree (higher degree accepted) Social Work (MSW)

      Required Work Experience:

      • 2 years case management experience (paid or internship) with at least one (1) year working with individuals experiencing homelessness within the last three (3) years
      • 1 year of supervisory, program development, and/or grant management experience

      Licenses and Certifications:

      • Licensed Master Social Worker in the state of Texas (Upon Hire)
