      Position Purpose: Partner with wide range of stakeholders to design partnerships that “bridge the gap” between healthcare and social services. This national role works with plans across multiple states and has three key areas of focus: (1) Establish and maintain partnerships with community, business, and government agencies to achieve SDOH and community health goals. (2) Support health plan activities related to partnership design, implementation, and performance improvement to improve outcomes for members and communities we serve – including engagement, outcomes, and access to services. (3) Provide coaching and training across multiple levels of the organization, using expertise, influence, and communication skills to enable strong partnerships and outcomes.

      ***approximately 50% travel required for position***

      Partnerships & Strategy Design:
      *Source, establish, and maintain strategic partnerships to improve whole health outcomes – for example, with local, regional, and national agencies focused on food, transportation, housing ecosystems e.g. continuum of care agencies.
      *Identify emerging approaches, innovative programming, and policy opportunities to amplify and enhance community supports and to improve access and health.
      *Develop and/or deploy strategies to engage communities in co-developing partnership models – especially with historically and currently marginalized communities

      Performance Management
      *Advise stakeholders in contract development and review of performance and data to prioritize actions and drive ongoing improvements
      *Support design and implementation of outcomes-based agreements
      *Provide high-level recommendations to plans and partners to continuously improve partnership and shared goals, e.g. increase member and community engagement, strengthen social care coordination, improve health outcomes

      Coaching & Training
      *Provide consultation to staff from plans, shared services, and partners in designing partnership strategies (across all levels of the organization); effectively organize information and share with multiple audiences and partners
      *Educate staff and partners on available tools and reports to inform strategy and identify opportunities for improvement
      *Build strong cross-functional relationships with shared services and markets to implement strategies and partnerships

      *Support provider performance improvement strategy at the plan and provider levels
      *Analyze and interpret cost and quality data to interpret performance insights and prioritize provider actions
      *Educate providers on the use of tools and interpretation of data and identify opportunities for improvement
      *Understand member population and provider variation to effectively support providers with performance improvement
      *Provide high-level recommendations to providers regarding opportunities to optimize clinical workflow to increase member outreach and care coordination
      *Develop relationships with internal partners and key accounts

      Education/Experience:

      • Bachelor’s Degree in Healthcare Administration, Business Administration, or similar field or equivalent experience; Master’s degree preferred.
      • 5+ years of experience in managed care, value-based contracting, or clinical experience.
