      What to Expect (Job Responsibilities):
      - Partner with patients and care management team to achieve shared goals
      - Conduct patient outreach through various communication channels to schedule appointments and tests
      - Document patient information accurately in the Electronic Medical Record (EMR)
      - Monitor patient outcomes related to preventative screening and chronic disease management
      - Identify barriers to care adherence and collaborate with nursing staff and department leadership
      - Assist patients in completing necessary forms for individualized care plans
      - Facilitate pre-visit planning and patient reminders

      What is Required (Qualifications):
      - Graduate of an accredited Medical Assistant Program
      - Proficiency in Microsoft Office applications
      - Strong interpersonal, customer service, and communication skills
      - Excellent time management and organizational abilities
      - Current American Association Medical Assistants (AAMA) certification or equivalent
      - Current BLS and CPR certifications

      How to Stand Out (Preferred Qualifications):
      - Previous experience in outpatient ambulatory care setting
      - Experience in supporting clinical quality programs such as HEDIS, Care Coordination, or Chronic Disease Management
