      What to Expect (Job Responsibilities):
      - Conduct an average of 6 patient home visits per day
      - Verify Primary Care Provider details and confirm patient allergies
      - Perform Medication Reconciliation to ensure accurate medication lists
      - Administer the Annual Wellness Visit (AWV) Questionnaire and record vital signs
      - Carry out up to 6 Preventive Screenings and document results

      What is Required (Qualifications):
      - Minimum of 1 year of experience as a Medical Assistant
      - At least 6 months of experience in Internal Medicine or Family Medicine
      - Medical Assistant Diploma or Board Certification
      - Active driver's license and willingness to travel extensively throughout southern and east-coast states
      - Proficiency with EMR systems and availability from 8am-5pm, Monday - Friday

      How to Stand Out (Preferred Qualifications):
      - Experience with Annual Wellness Visits and home visits
      - Geriatric care experience
      - Fluency in multiple languages, including Spanish, Korean, Vietnamese, and Mandarin
