What to Expect (Job Responsibilities):
- Conduct comprehensive assessments of patients' medical, psychological, and social needs
- Assist in developing and implementing personalized care plans in collaboration with a multidisciplinary team
- Educate patients on managing chronic diseases, including medication adherence and lifestyle modifications
- Facilitate communication between patients, caregivers, and healthcare team members for seamless care coordination
- Monitor RPM data for trends and escalate findings to appropriate care team members

What is Required (Qualifications):
- Certified Medical Assistant (MA) credential required
- Minimum of 2 years of experience in patient care, care coordination, or chronic disease management
- Proficiency in electronic health records (EHR) systems
- Excellent interpersonal and communication skills for effective patient education
- Strong organizational and multitasking abilities with attention to detail

How to Stand Out (Preferred Qualifications):
- Certification in disease management, case management, or patient education
- Experience with Chronic Care Management (CCM) or Remote Patient Monitoring (RPM) programs
- Familiarity with RPM devices and data monitoring tools
