What to Expect (Job Responsibilities):
- Act as a liaison for members by following up after appointments to ensure quality care and health outcomes
- Communicate lab results and other findings to members and Primary Care Providers as necessary
- Assist with case management and referrals for at-risk patients
- Perform administrative tasks including documentation and updating patient records
- Facilitate care coordination by communicating with primary care physicians and nursing staff

What is Required (Qualifications):
- High School Diploma or GED
- Minimum of 1-3 years of clinical experience in settings such as family practice or primary care
- Graduate of an accredited Certified Medical Assistant program
- Strong customer service and communication skills, both written and verbal
- Ability to lift up to 50 lbs and demonstrate attention to detail

How to Stand Out (Preferred Qualifications):
- More than 1 year of experience as a Certified Medical Assistant
- Familiarity with electronic medical record systems, preferably Epic
- Experience working with underserved populations facing socioeconomic barriers
- Bilingual in Spanish, Korean, Mandarin, Cantonese, or Vietnamese
