- Answering calls and dealing with all aspects of patient facing care including booking appointments, greeting, and directing patients.
- Relaying information and queries between patients and clinicians, ensuring clear and concise documentation.
- Care navigation (signposting patients to other appropriate services).
- Processing online consultations
- Actioning and sending tasks to clinical team
- Liaising with wider colleagues in primary and secondary care
- Dealing with sensitive information always adhering to confidentiality policy
- Full working knowledge of Emis for all functions required for the role
- Book appointments on Emis and signpost to other services appropriately
- Fill up appointments correctly for varied clinicians by signposting correctly
- Input prescriptions accurately
- Chase up daily DNAs up & do tasks efficiently
- Undertake other GP related tasks such as patient registrations, referrals & coding
- Register new patients educating them on the practice booking options
- Identify and understand patient needs and requirements, through appropriate questioning
- Maintain high standards of patient confidentiality.

- Maintain a clean, tidy, effective working area at all times

Job summary
We have an exciting opportunity
We are recruiting a Care Navigator to join our busy, friendly reception team.
The successful applicant will be highly motivated, adaptable and be able to work under pressure.
Our Care Navigators work closely with our clinical team and wider colleagues in person and via email and tasks, therefore excellent communication skills and strong I.T skills are essential for the role.
Experience working in a GP Surgery is essential.
The position is permanent and subject to a 6-month probation period. Hours would be set but flexibility is essential as there would be a requirement to provide annual leave/sickness cover when needed.
The ideal successful candidate will be flexible to cover holidays, absences and undertake any administrative works to suit the needs of the practice while delivering an empathic and professional service delivery to patients.

About us
We are a friendly supportive and busy General Practice with approximately 7300 patients.
We are an established team with vast experience of dealing with patients and clinical queries. We have two very experienced GP's, Practice Nurses, on site clinical Pharmacists and many other Healthcare professionals.
We are also part of the Primary Care Network (PCN) encompassing 8 local General Practices. The PCN provides guidance and support and shares a variety of resources and always looks out for the development of new services to improve patient care.
