What to Expect (Job Responsibilities):
- Provide comprehensive primary care for a small panel of home-bound, medically complex older adults
- Evaluate and treat patients in their homes while coordinating care with team members and specialists
- Participate in weekly interdisciplinary team meetings to discuss patient care
- Collaborate closely with the Medical Director and other healthcare professionals
- Ensure high-quality medical care that meets the standards of the Johns Hopkins Health System

What is Required (Qualifications):
- Active Nurse Practitioner Licensure in the State of Maryland
- Must possess a working vehicle for traveling to patients' homes
- CPR Certification is required
- Full-time availability for 40 hours per week, from 8am to 5pm
- Experience in palliative care and/or geriatric care is preferred

How to Stand Out (Preferred Qualifications):
- Certification in palliative care or geriatric nursing
- Previous experience working with home-based patients
- Strong communication and interpersonal skills
- Ability to work independently and as part of a collaborative team
