      What to Expect (Job Responsibilities):
      - Conduct in-home assessments and some outpatient clinical work on health plan members
      - Obtain comprehensive history, perform physical exams, and conduct medication reviews
      - Discuss end-of-life care with patients and families as necessary
      - Collect laboratory specimens in the home setting when appropriate
      - Communicate assessment findings to inform the primary care physician of potential gaps in care

      What is Required (Qualifications):
      - Master's degree from an accredited Nurse Practitioner (NP) or Physician Assistant (PA) program
      - Active California state Nurse Practitioner or Physician Assistant license
      - Active RN License and Furnishing number (NP only, must acquire upon hire)
      - Active Nurse Practitioner Board Certification/Physician Assistant Certification
      - Valid California driver license and current automobile insurance

      How to Stand Out (Preferred Qualifications):
      - One (1) year of prior clinical or home care experience
      - Previous experience with electronic medical records (EMR)
      - Experience in the care of older adult (geriatric) patients
      - Excellent administrative, organizational, and communication skills
