Key Responsibilities:

• Provide telehealth consultations and care management for patients with multiple chronic conditions (e.g., diabetes, hypertension, heart disease, COPD) via phone, video, and other telecommunication platforms.
• Lead and supervise a team of field nurses, coordinating in-home visits to assess, monitor, and support patient care plans.
• Review and analyze patient health data, including lab results, diagnostic reports, and nursing assessments, to formulate care plans and treatment recommendations.
• Develop and implement individualized care plans for patients, ensuring they align with evidence-based best practices and patient needs.
• Collaborate with other healthcare providers, including specialists, primary care physicians, and multidisciplinary care teams, to ensure comprehensive and coordinated care.
• Provide patient education and support in managing chronic conditions, including medication management, lifestyle modifications, and symptom management.
• Monitor and adjust treatment plans as necessary to improve patient outcomes and manage exacerbations or complications.
• Act as a mentor and resource for the nursing team, offering guidance and support for home visits, patient communication, and clinical decision-making.
• Ensure timely documentation of patient interactions, assessments, and care plans in the electronic health record (EHR).
• Maintain up-to-date knowledge of medical advancements, telehealth technologies, and regulatory requirements related to nurse practitioner practice.
• Ensure adherence to all federal and state regulations, including HIPAA compliance and licensure requirements.

Qualifications:

• Active Nurse Practitioner licensure in North Carolina in good standing.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (AGNP), or other relevant NP specialty.
• Minimum of 3 years of experience as a Nurse Practitioner, with a focus on chronic disease management preferred.
• Strong clinical knowledge and experience in managing patients with multiple chronic conditions.
• Experience in telehealth or virtual care delivery is highly desirable.
• Leadership experience, including the ability to supervise, mentor, and support nursing staff.
• Excellent communication skills, both verbal and written, with the ability to engage patients in meaningful care conversations.
• Proficiency in using electronic health record (EHR) systems and telehealth technology platforms.
• Strong organizational and time-management skills, with the ability to work independently and prioritize tasks in a fast-paced environment.
• Compassionate, patient-focused approach to care with a commitment to improving patient outcomes.
