What to Expect (Job Responsibilities):
- Conduct comprehensive in-home history and physical assessments for enrolled members
- Capture and document annual diagnoses accurately to ensure Medicare risk adjustment
- Collaborate with primary care providers and specialists to develop member-specific medical care plans
- Educate and counsel members and their families on identified health conditions
- Provide consultations for medically complex members and ensure timely appointments and documentation

What is Required (Qualifications):
- Must be a Nurse Practitioner or Physician Assistant with relevant educational qualifications
- Current national certification as a Nurse Practitioner or State certification as a Physician Assistant
- Strong proficiency in MS Office products such as Excel, Word, Outlook, and PowerPoint
- Excellent organizational skills with the ability to multitask and prioritize projects
- Current unrestricted driver's license

How to Stand Out (Preferred Qualifications):
- Previous experience as a Registered Nurse, Nurse Practitioner, or Physician Assistant, particularly with geriatric and disabled populations
- Two or more years of clinical experience in a healthcare setting
