What to Expect (Job Responsibilities):
- Conduct comprehensive in-home history and physical assessments to identify active and chronic disease conditions
- Capture and document annual diagnoses accurately for Medicare risk adjustment
- Develop medical care plans in collaboration with primary care providers and specialists
- Educate and counsel members and families on identified conditions during assessments
- Provide consultations for medically complex members and ensure timely documentation

What is Required (Qualifications):
- Must be a licensed Nurse Practitioner or Physician Assistant with relevant educational background
- Proficient in MS Office products including Excel, Word, Outlook, and PowerPoint
- Strong organizational skills with the ability to multitask and prioritize projects
- Current national certification as a Nurse Practitioner or State certification as a Physician Assistant
- Current and unrestricted driver's license

How to Stand Out (Preferred Qualifications):
- Previous experience as a Registered Nurse, Nurse Practitioner, or Physician Assistant, especially with geriatric or disabled populations
- Two or more years of clinical experience
