What to Expect (Job Responsibilities):
- Conduct in-home assessments for health plan members, including comprehensive history and physical exams
- Engage in end-of-life care discussions with patients and families
- Collect laboratory specimens in home settings when appropriate
- Identify diagnoses for active medical management and care coordination
- Communicate assessment findings to primary care providers to address gaps in care

What is Required (Qualifications):
- Active California state Nurse Practitioner or Physician Assistant license
- Master's degree from an accredited Nurse Practitioner or Physician Assistant program
- Ability to communicate positively and effectively with others
- Strong problem-solving, organizational, and time management skills
- Valid California driver license and current automobile insurance

How to Stand Out (Preferred Qualifications):
- One (1) year of prior clinical or home care experience
- Experience in the care of older adult (geriatric) patients
- Previous experience with electronic medical records (EMR)
