      What to Expect (Job Responsibilities):
      - Conduct in-home assessments and some outpatient clinical work for health plan members
      - Obtain comprehensive patient history, perform physical exams, and conduct medication reviews
      - Facilitate end-of-life care discussions and laboratory specimen collection when appropriate
      - Identify diagnoses for active medical management and communicate findings to primary care providers
      - Provide education and support to patients, families, and caretakers, ensuring proper follow-up and resource referrals

      What is Required (Qualifications):
      - Master’s degree from an accredited Nurse Practitioner or Physician Assistant program
      - Active California state Nurse Practitioner or Physician Assistant license
      - Active RN License and Furnishing number (for NP only)
      - Active Nurse Practitioner Board Certification/Physician Assistant Certification
      - Valid California driver license and current automobile insurance

      How to Stand Out (Preferred Qualifications):
      - One (1) year of prior clinical or home care experience
      - Previous experience with electronic medical records (EMR)
      - Experience in caring for older adult (geriatric) patients
      - Strong problem-solving, organizational, and time management skills
      - Ability to communicate effectively and professionally with patients and team members
