Bachelor’s degree in nursing plus ten (10) years of clinical care management, including complex patient populations & utilization review experience to include five (5) years of supervisory or leadership experience.
Current Registered Nursing License with the ability to obtain licensure in the state of Arkansas.
Experience in developing and leading care coordination programs.
Strong leadership and team management skills with proven ability to analyze data and implement quality improvement initiatives.
Excellent communication and interpersonal skills to collaborate with diverse stakeholders.
Knowledge of healthcare regulations and reimbursement models.
Knowledge and skills to utilize various Care Management platforms including but not limited to: InterQual/MCG criteria.
Master’s degree in nursing (MSN), healthcare administration, or related field plus ten (10) years of experience in healthcare management, Social Work, leading Social Work teams and/or leading patient access teams.
Experience with Epic and Workday.
Case Management Certification (CMC).
Oversee the coordination of patient care across multiple services and providers, ensuring timely appropriate interventions and comprehensive care plans tailored to individual patient needs.
Establish and monitor clinical protocols, ensuring care managers adhere to best practices for patient assessment, care planning, and intervention delivery.
Lead efforts in ensuring that social workers and care management teams address the emotional, social, and psychological needs of patients.
Advocate for patients by providing access to necessary resources such as financial assistance, community support services, and post-discharge planning.
Partner with other departments within the organization, including clinical teams, finance, and operations, to facilitate seamless patient care, smooth transitions and minimize readmissions.
Oversee the development of individualized discharge plans that include follow-up care, community resources, and post-hospital services.
Support the Utilization Management Committee. Collaborate with the Chief Medical Officer and Physician Advisor to identify utilization trends.
Work closely with financial department in denials management.
Develop relationships with the payer community and identify opportunities to collaborate in Utilization Management and discharge planning.
Develop and execute comprehensive care management strategies aligned with organizational goals, including patient population needs, quality metrics, and financial considerations.
Identify and allocate resources to address patients' needs, including community-based programs, housing, transportation, and financial support.
Monitor and manage budgets related to care management programs, ensuring efficient use of hospital resources.
Supervise and provide leadership support to the care management team, including social workers, case managers, and other staff members.
Conduct regular performance evaluations, provide coaching, and facilitate professional development for team members.
Ensure staff is trained in best practices and hospital policies related to care management and social work.
Foster effective communication and collaboration between the care management team, physicians, nursing staff, and other hospital departments.
Act as a liaison to ensure all parties involved in patient care are aligned on treatment goals and progress.
Ensure compliance with hospital policies, regulatory standards, and ethical practices related to care management and social work.
Monitor and evaluate care management processes, providing recommendations for improvement to hospital leadership.
Analyze patient data to identify quality gaps and implement interventions to improve patient outcomes, including monitoring key performance indicators (KPIs).
Develop and implement strategies to improve patient satisfaction.
Regularly assess care management practices, leveraging data to inform process improvements and enhance service delivery.
Collect, analyze, and report on key performance indicators (KPIs) related to care management, patient outcomes, and team performance.
Present data to hospital leadership and collaborate on action plans to improve care management services.
Other duties as assigned.

The Director of Care Management is accountable for the leadership of the hospital's care management department, overseeing the utilization review team, and providing supervision and support to social workers and other staff within the department. This position ensures the efficient coordination of patient care, optimization of resource utilization, and a seamless patient experience throughout their hospital stay, while maintaining compliance with clinical protocols and regulatory standards. The Director will lead a collaborative team across various departments to enhance patient outcomes, advocate for patients, and effectively manage the discharge process. This role reports directly to the Associate Vice Chancellor, Chief Clinical Access Officer.
