      Your Specific Responsibilities:

      This is a professional OPS registered nurse position in the field of public health responsible for providing family centered care coordination to a population of HIV positive pregnant women and HIV exposed/positive babies in Area 15 (St Lucie, Martin, Okeechobee, Indian River). This position will be the lead Hug Me Nurse for Area 15. This includes assessing, planning, implementing, coordinating, monitoring and evaluating services to meet the needs of the identified population. Performs work in an area geographically removed from direct supervision, i.e. Home, hospital, doctor’s office, clinic settings, labs, etc. The incumbent will comply with all DOH policies and procedures regarding the security of confidential information. The incumbent works directly under the supervision of the Community Health Nursing Consultant. Use of personal vehicle may be required. Duties include, but are not limited to:

      Accepts primary responsibility for coordinating the management of care and ensuring follow-up by:

      a. Working collaboratively with other disciplines, e.g. Healthy Start, TOPWA, Ryan White case management, HIV Surveillance, DIS, OB’s, pediatricians, ID specialists and Pediatric ID specialists in assessing and meeting the physical, psychological, social, emotional, and rehabilitation needs of HIV exposed infants.

      b. Providing nursing consultation to children and their families, other team members and the community, regarding management of the health needs of the patients, assuring continuity of care.

      c. Reviewing and assessing laboratory and other diagnostic tests and reports.

      d. Planning and implementing actions, identified along with the family, to meet the needs of the patient. Monitoring and evaluating outcomes and goal achievement.

      e. Completing established assessments and care coordination plans

      f. Teaching parents/guardian’s skills and information needed to care for the child in the home as outlined in CDC guidelines

      h. Identifying and utilizing appropriate DOH and other community resources to meet the needs of the children and their families.

      i. Providing education and support to families with children with special health care needs to increase the family’s capacity to become the child’s primary care coordinator.

      i. Making home, hospital visits for coordination and continuity of patient care. Attending client staffing’s as needed.

      j. Assisting families and individuals to cope with crisis situations such as chronic illness, surgeries, and terminal illness.

      k. Coordinating the provision of care utilizing the available ID specialty pediatric care

      l. Providing health education to women of childbearing age, those who know their HIV status and those whose status is unknown, regarding the importance of being tested prior to delivery and prevention of perinatal transmission

      m. Educate HIV infected mothers about the importance of ongoing follow up care and adherence to medication therapy for their exposed infants and themselves

      n. Meet with HIV infected mothers before they are discharged from the hospital

      o. Ensure that sufficient medication is available for the infants upon discharge

      p. Link both the HIV infected mother and the exposed infant to appropriate follow up care, to include medical transportation

      q. Provide home visits when necessary to retain women and their exposed or infected infants in care; to include visits with medical providers

      r. Submit Ryan White reports

      s. Enter services & data into CareWare & HMS

      t. Document in medical record at the time of service

      u. Attend all staff, program and department/agency meetings as required by supervisor

      v. Maintain and update the Mama Bear and Baby Bear spreadsheets & monthly data spreadsheets

      w. Complete weekly assignments schedules, monthly case management reports, and quarterly Hug Me reports, HRSA data requests and annual RSR.

      x. Educate OB and pediatric providers on the latest HIV treatment guidelines put out by CDC

      y. Educate pregnant women and post-partum women on the importance of choosing a family planning method, assist women in obtaining an appropriate method of birth control

      z. Provide care coordination to HIV positive infants up to 24months of age.

      Documents case notes and encounters in HMS. Maintains a file for each client with required documentation as per DOH guidelines. Completes all case management documentation, EARs, etc. within approved DOH guidelines. Participates in all assigned staff meetings, community meetings related to HIV and perinatal prevention. Attends outreach events as directed.

      Co-Chairs the monthly Mama Bear meetings, coordinates mama bear meetings/calls within Area 15

      Other duties as assigned, including, but not limited to, disaster preparedness activities and outreach events.

      Other related duties as deemed necessary to ensure provision of services to all positive women, exposed and/or positive babies and women at risk for disease.

      Maintains client confidentiality and security in accordance DOHP50-10-10 Information Security & Privacy Policy Confidential Information. Will be assigned security level 3.

      Required Knowledge, Skills, and Abilities:

      Knowledge of prenatal, postnatal and interconceptional care, 076 protocol, Computer literate, Community resources

      Qualifications:

      Minimum:

      • Active Florida (or multistate) RN license
      • Willing to work evenings and/or weekends
      • Willing to use your own vehicle for travel
      • Have a valid Florida driver’s license
      • At least 1-2 years of maternal child health nursing experience
      • Willing to discuss family planning, HIV and STI’s
      • Willing to work in a special needs shelter
      • Willing to do home visits
      • Willing to work in high crime areas
      • Experience working people living with HIV/AIDS
      • At least 2-3 years’ experience demonstrating proficiency in Microsoft Office & email

      Preferred:

      • Fluent in English/Spanish or English/Creole
      • At least 1 year working with persons living with HIV/AIDS
      • At least 1 year previous DOH experience
      • Already trained in HIV 500/501
      • At least 1 year of case management experience (or equivalent)
