- Provide support to for Diabetes Care: Deliver support, education, and advice to individuals living with diabetes, ensuring optimal outcomes across the adult population and types of diabetes.
- Behaviour Change Management: Apply the principles of behaviour change to empower patients to make sustainable lifestyle changes that improve their diabetes control, reduce complications, and enhance quality of life. This includes the use of motivational interviewing and other therapeutic techniques.
- Education & Training: Deliver high-quality diabetes education to patients, their families, and colleagues, ensuring a holistic approach to care. Facilitate group and individual educational sessions on topics such as medication management, blood glucose monitoring, nutrition, exercise, and stress management. Key Requirements:
- Expertise in Behaviour Change (e.g., Motivational Interviewing, CBT, or other behaviour management techniques).
- Excellent Communication Skills, with the ability to build rapport and inspire patients to adopt and maintain behaviour changes.
- Commitment to Continuous Learning, keeping up-to-date with the latest research and developments in diabetes care.
- NVQ/Diploma Level 4 in Health and Social Care or equivalent level of knowledge, skills and experience
- Care Certificate or willingness to undertake this within induction period
- Knowledge of planning and completing existing patient support assessments
- Demonstrate awareness of insulin dependent diabetes
- Clear understanding of supporting and guiding patients with aspects of their care
- Clear understanding of patient needs to support self-management in the community
- Knowledge of a range of software programmes including Microsoft Word, Excel and Outlook
- Knowledge of completing risk assessments
- Knowledge of policies, procedures and practices relating to the INCS
- Knowledge of techniques used to support and motivate behaviour change in individuals
- Knowledge of relevant legislation e.g. Mental Health Act (1983), Mental Capacity Act (2005), Safeguarding Adults and Deprivation of Liberty (DOLS)
- Knowledge of SystmOne
- Experience of completing patient assessments in a variety of settings including patients' own home or Treatment Room
- Experience of working in a health and/or social care setting
- Experience of undertaking risk assessments
- Experience of keeping accurate records and inputting data into relevant database systems
- Experience of working with people with a range of health needs and adults with complex health care conditions
- Experience of working with people with diabetes and other nursing needs e.g. wound care
- Experience of liaising and working with multidisciplinary teams or care teams

In this role, you will play a pivotal part in supporting individuals with diabetes to achieve better health outcomes through tailored education, guidance, and behaviour change interventions.
Additionally, this post requires that you have the ability to travel between locations to carry out your work.
This post is based within the East locality cluster.
