      Job Title:Outreach Nurse/Allied Healthcare Professional for the Haringey Homeless Health Inclusion Team, Haringey GP Federation

      Accountable to: Clinical Directors for Haringey Primary Care Networks and Chief Executive Office for Haringey GP Federation

      Line manager: Integrated Services Manager Haringey GP Federation / GP Clinical Lead

      Hours of work: Part time

      Remuneration: £44,000 - £52,000, depending on experience. Salary is pro-rata.

      Contract length:Fixed term until 31/03/26 with option to extend

      The Haringey GP Federation is looking for an experienced clinician who would be able to work within their professional boundaries for the successfully established Haringey Homeless Health Inclusion Team.

      As an Outreach Clinician you will provide holistic medical care for patients experiencing homelessness and rough sleeping in Haringey.

      The role will include:

      Clinical outreach shifts for patients living on the streets and in emergency accommodation. Outreach is a joint service with the teams care navigator and currently takes place four days a week. There is no lone working.

      Participation (and occasional chairing) at the weekly multidisciplinary meeting with allied services such as local drugs and alcohol teams.

      Weekly drop-in sessions at the day centre for patients experiencing homelessness.

      Contribution to the services weekly GP led health inclusion clinic.

      Working with the Haringey Homeless Health Inclusion Team on service maintenance and service build

      Working with the Haringey Homeless Health Inclusion Team on stakeholder and community engagement by contributing to local and national presentations

      • Relevant registration as a Paramedic/Nurse/Relevant clinical position (minimum 1 year post registration).
      • AND:
      • DBS clearance
      • Current and valid certification
      • COVID-19 Vaccination Status

      Desirable

      • Prescriber certification
      • Minor Illness certification
      • Member of College of Paramedics
      • Evidence of Continuing Professional Development activities
      • Basic Life Support
      • Diploma in Primary and Urgent Care
      • Experience working with patients experiencing homelessness
