JOB OPPORTUNITY: Palliative & Geriatric Medicine Physician

The Hospital of Central Connecticut (HOCC) seeks a full-time Palliative & Geriatric Medicine Physician to: provide geriatric and palliative care consultations to adult patients in the hospital and outpatient setting; conduct assessments and develop personalized care plans for geriatric patients; collaborate with other healthcare professionals, including physicians, nurses, social workers, and pharmacists, to ensure a holistic approach to patient care; educate and support patients and their families regarding medical conditions, treatment options, and palliative care strategies; participate in and contribute to quality improvement initiatives aimed at enhancing geriatric care and palliative services; provide leadership in ethical decision-making and advance care planning discussions. In addition to working from HOCC at 100 Grand Street, New Britain, CT 06050, this position will work from one or more of the following outpatient clinic sites approximately once per week: 280 South Main Street, Cheshire, CT 06410; 201 North Mountain Road, Plainville, CT 06062; and 98 Main Street, Southington, CT 06489.

The minimum requirements for this position are: a U.S. M.D. or D.O. degree or its foreign equivalent; completion of a U.S. medical residency in Internal medicine and U.S. medical fellowship program in Geriatric and Palliative medicine; board eligibility or certification in Internal medicine, Hospice and Palliative medicine, and Geriatric medicine; and the possession of or eligibility for an unrestricted Connecticut medical license.
