- Assess, diagnose, and manage patients with urgent health queries within your scope of practice.
- Provide face-to-face consultations to deliver timely and effective care.

Job description
Job responsibilities
Diagnosing and treating patients presenting with minor illness or minor injuries
Assess, diagnose, plan, implement and evaluate treatment/ interventions and care for patients presenting with an undifferentiated diagnosis for all ages
Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly, making any necessary referrals to other members of the primary health care team
Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
Advise patients on general health care and minor ailments with referral to other members of the primary and secondary health care team as necessary
Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care
Diagnosing and treating patients visited at home
Triage and visit patients requesting a home visit by a health care professional making any necessary referrals to other members of the primary health care team
Be responsible for the visits to the nursing and residential homes, liaising with homes management to arrange routine and acute visits to residents as required
Ensure clinical practice is safe and effective and remains within boundaries of competence, and to acknowledge limitations
Advise patients on general health care with referral to other members of the primary and secondary health care team as necessary
Clinical Practice for above roles
Practice autonomously using a patient centred partnership approach within their scope of extended practice
Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly
Acts as an advocate for the patient to ensure healthcare needs are met consistent with patients informed choices
Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long term condition
Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug based treatment methods into a management plan
Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence based practice, national and practice protocols and formularies and within scope of practice, after training completed.
Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care through collaborative working using a variety of health promotion and disease prevention strategies in line with local and national polices
Compassionately identify patients with drug and alcohol problems and effectively prioritise the actions needed to address the issues raised
Plan appropriate review ensuring the patient understands how and when to access further help or assistance or referral (safety netting)
In collaboration with the multi-professional team sensitively apply the principles of end of life care to cancer and non-cancer illness
Advocate the public screening programmes to individuals as appropriate both male and female and ensure findings are communicated effectively
Advocate immunisation programmes for children, adults and travel, and direct patients to other members of the team (Practice Nurses) for implementation
Mental Health and Psychological Care
Work in a collaborative and shared care approach between primary and secondary care
Understand the impact of disease-related distress to psychological and psychiatric conditions
Encourage self-management of mental health including acceptance of illness, managing symptoms, personal motivation and encourage adherence to treatment regimes
Paediatrics
Demonstrate an understanding of the importance of multiagency working and the principles of information sharing
Ensure that parents or carers, children and young people receive information advice and support to enable them to:
- Manage minor illnesses themselves using community pharmacists and triage services where appropriate
- Access appropriate services when necessary
- Access support groups
Be aware of disease prevention wellbeing and safety in children and adolescents
Learning disability
Ensure that when patients with learning disabilities also have another condition that special attention is made to their specific needs and their care is tailored to these
