What to Expect (Job Responsibilities):
- Conduct in-home assessments on health plan members, including medical history, physical exams, and medication reviews
- Identify diagnoses for care management and active medical treatment
- Educate members on disease processes, medication compliance, and health topics
- Communicate assessment findings to inform primary care providers of potential gaps in care
- Ensure compliance with HIPAA regulations and maintain the security of protected health information (PHI)
What is Required (Qualifications):
- Active, unencumbered Nurse Practitioner (NP) or Physician Assistant (PA) license to practice in Ohio
- Ability to obtain DEA licensure/prescriptive authority post-hire as applicable
- NP certification from ANCC, AANP, or AACN, or NCCPA certification for PAs
- Minimum of 1 year of clinical experience or 3+ years as a Registered Nurse
- Access to reliable transportation to visit patient sites within a designated area
How to Stand Out (Preferred Qualifications):
- Experience in Gerontology, Cardiology, Internal Medicine, or Endocrinology
- Previous home care or home visit experience
- Proficient in computer skills and internet navigation
- Strong administrative and organizational skills with effective communication abilities with seniors and their families
