      What to Expect (Job Responsibilities):
      - Establish and maintain effective relationships with patients, caregivers, and healthcare team members
      - Perform initial comprehensive assessments and ongoing care for newly enrolled patients
      - Develop and update patient management care plans based on changes in condition
      - Participate in clinical after-hours on-call program on a rotational basis
      - Conduct post-discharge visits to ensure patient stability aligns with their care goals

      What is Required (Qualifications):
      - Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the National Commission on Certification of Physician Assistants (NCCPA)
      - Current, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) licensure in the State of Practice
      - Active, unrestricted RN licensure in the State of Practice (for Nurse Practitioners)
      - Access to reliable transportation and compliance with the employer's Motor Vehicle Safety policy

      How to Stand Out (Preferred Qualifications):
      - 2+ years of clinical experience in primary care, home-based medical care, geriatrics, or similar settings
      - Knowledge of long-term care, community resources, and cost-effective patient care delivery systems
      - Familiarity with CPT, ICD-10, and HCPC codes
      - Understanding of clinical standards of care and NCQA requirements
