The goal of the Health Home model of service delivery is to enhance coordination of medical, behavioral health and specialty care, monitor functioning across multiple domains and to build linkages to other community and social supports for children/adolescents with chronic illnesses, severe emotional disturbances and/or complex trauma. The position will oversee and coordinate access to all of the services, supports and benefits (i.e. medical, behavioral health, specialty, educational, social, etc.) an enrollee requires in order to improve overall functioning and maintain stability through regular visits in the community and related documentation requirements as dictated by New York State's Department of Health (NYS DOH). The HHSC program serves eligible individuals who reside in NYC and lower Westchester County. As such, this position will be expected to complete home and community visits where eligible individuals and enrollees reside based on their availability.

• Bachelor’s Degree in a health or human services field from an accredited college or university, min of 2 years' experience working with children, adolescents and families in the field/community.
• Position will require home visits and community based service provision. Valid NYS driver's license preferred but not required.
• Working knowledge of children's system of care, community based resources and public benefits.
• Ability to travel to various locations for face-to-face meetings with enrollees outside of traditional business hours (8:30-5 pm) as per enrollee's availability/needs. Weekly schedule will be flexed and adjusted to accommodate evening visits. Schedule will be maintained by the department.
• Ability to provide and experience providing culturally sensitive, person-centered community based services to children, adolescents and families.
• Fluency in Spanish highly preferred.
• Experience with EHRs, databases, HCS and CANS -NY preferred.
