What to Expect (Job Responsibilities):
- Conduct outbound calls to eligible patients to introduce the @HOME program and encourage participation in home-based care
- Foster relationships with patients and their caregivers to enhance engagement in the program
- Educate patients about various care services accessible through their health plan or primary care provider
- Collaborate with care teams to identify innovative ways to connect with patients
- Operate independently in a fast-paced environment while demonstrating excellent communication and problem-solving skills

What is Required (Qualifications):
- Track record of success in health-care-related engagement, sales, and/or marketing
- Strong ability to independently build relationships with patients and their caregivers
- Understanding of the Medicare and Medicare Advantage programs
- Proficient computer skills, including navigation of Gmail, MS Word, Google Docs, Excel, and Google Sheets
- US Work Authorization is required

How to Stand Out (Preferred Qualifications):
- Bilingual (English and Spanish) is a plus
- Demonstrated passion for improving the lives of older adults
- Experience in a healthcare environment with a focus on patient engagement
