Job Description

- To take calls from GPs about assessment or admission of patients ascertaining which speciality the GP is wanting to speak with then transferring/conferencing the call with the relevant speciality doctors within the trust.
- To ensure where feasible, patients that require admission into the trust are allocated to the most appropriate ward/department for their care.
- Monitor A&E activity and waiting times using the symphony system, escalating to the Patient Flow Manager, out of hours, when patients are coming up to 12 hr breaching.
- To cover switchboard when needed to put the crash call out.
- Take bookings for the relatives rooms out of hours and blocking out the relevant room on the outlook calendars and providing the swipe cards to enable the relatives to gain access, also collecting the swipe cards back into the office at around 11am over the weekend.
- Carry out general office duties including photocopying, faxing and scanning, disposing of confidential/sensitive material in the relevant bags, organising of the white display boards. To use, maintain handle and store equipment and materials efficiently, effectively and safely.
- To be flexible in covering short falls within the team to cover for colleague absences where able to.
- Participate in the general cleanliness and tidiness of your work environment.
- Maintain the bed management system on symphony acknowledging when a bed needs to be booked and marking the request to the relevant ward area that is requested for the patient.
- Take repatriation referrals from external trusts for patients needing to be brought back into Mid Yorks NHS trust ensuring the patient has been accepted by the relevant consultant on call and taking time to make sure all the correct patient information is collated into the repatriation folder.
- Obtain regional bed states as and when requested of us when escalation is needed regarding bed availability during extreme pressures.
- To book ambulance transport as and when needed for those patients that have been waiting at home for a bed to become available through the hospital transport service or YAS dependant on the patients needs/mobility/requirements.
- Undertake calls from the public looking for relatives within Mid Yorks when the admissions office is unavailable to take the call.
- May be required to undertake training to be able to create accounts and give access to emeds and PPM+ to nursing staff/doctors including agency staff at times when admissions office are unable to do so.
- The above duties and responsibilities are not exclusive to the role and we are to undertake any other duties/jobs requested of us by any other staff/management/colleagues that are within our own limitations.

Job summary
An exciting opportunity has arisen for a full time Patient Flow Administrator to support the Patient Flow Team, on a permanent basis.
This is a rewarding and challenging operational role that will deal with complex and dynamic situations. Alongside handling phone calls and booking patient transport , the post holder will support the wider operational team by linking with a particular area of the hospital to improve communication within the hospital.
The ideal candidate will have experience of working in a large healthcare organization in an operational setting. This role would ideally suit a candidate who is looking to develop their skills and experience within this environment.

Main duties of the job
To assist in the co-ordination of emergency and elective admissions into Mid Yorkshire Teaching NHS Trust.
To collect, collate and monitor data surrounding bed utilization across the trust reporting to the Patient Flow Team. To assist colleagues and medical staff in the appropriate use of beds. To handle calls from GPs and other Health Care Professionals relating to assessment or admission of patients.
To provide regular, accurate and timely updates to the Patient Flow Team re: outstanding patients waiting to be admitted, A&E breach escalation and any other information passed to ourselves that needs to be escalated.
