- Bachelor's degree in behavioral health profession
- Minimum 3 years demonstrated experience working with substance-use disorder clients
- Strong communication skills, active listening skills, cooperation and teamwork
- Must possess the knowledge, skills and experience necessary to completely perform case management
- Demonstrate a commitment to diversity, equity, and inclusion through continuous development/training, modeling inclusive behaviors, and proactively managing bias
- Master’s degree
- LCADC, LPC, LCSW, LMFT, or Licensed Psychologist
- ASAM knowledge and experience with ASAM placement criteria
- Shift working,8:00am - 4:00pm,subject to change based on department needs.
- Monday – Friday
- Full-Time
- Screen for substance-use disorders and provide warm hand-off to substance-use treatment services
- Screen for social determinants of health and provide patient navigation services to community resources that address patient needs
- Educate patient on multiple pathways to recovery, treatment options and medications for opioid use disorder, MOUD,
- Serve as a navigator and patient advocate to help remove all barriers to recovery
- Collaborate with system and community-based partners to ensure coordination of care
- Attend and represent the Institute for Prevention and Recovery at state and county level stakeholder meetings

Job Overview:
The Patient Navigator for the Peer Recovery Program is responsible for effectively guiding patients with substance-use disorders through the healthcare system and resources needed to initiate and maintain recovery. The Patient Navigator will screen the individual for social determinants of health, determine the most appropriate services to address their needs and educate the patient on their options. The Patient Navigator will assist in removing any barriers associated with access to care. The Patient Navigator will manage follow-up appointments to coordinate continuation of care and linkages to community resources. The Patient Navigator will also work alongside the Recovery Specialist team to support the patient and ensure highest quality of care.
