What to Expect (Job Responsibilities):
- Facilitate timely and appropriate discharges and care transition support services through collaboration with providers, community service agencies, and other stakeholders
- Conduct brief functional needs assessments, develop service plans, and provide service coordination
- Provide assistance and support to the Oregon Behavioral Health Support Program and other employer staff
- Maintain and distribute information about the availability of community resources
- Assist and support Medicaid consumers during the care transition process
What is Required (Qualifications):
- Bachelor’s degree in a related field; Equivalent combination of education and/or work experience in related field may be substituted
- Qualified Mental Health Associate (QMHA) Certification required
- 2+ years of clinical experience
- Must reside in Oregon
- Travel is required for this position
